COPA Letter Request Form

I have read and understand the policies and deadlines outlined on the COPA Letter web page and the Prehealth Credential Service web page, which include, but are not limited to:

I must authorize the release of my test scores, AMCAS, AADSAS, and VMCAS application information to my Undergraduate Health Professions Advisor. (via AAMC, ADEA, AAVMC)
MIT Prehealth Advising has the right to view my test scores, transcripts, letters of recommendation, and personal statement, as well as release these items to COPA for use exclusively in writing the COPA Letter.
I understand that Prehealth Advising will be checking with the Office of Student Citizenship regarding any student conduct violations on my record.
To complete your COPA Letter Request you are required to submit the nonrefundable, annual Prehealth Credential Service Fee, which is $100. You will be directed to the payment page when you click submit below so you must have your credit card information ready.
