PURPOSE & SCOPE: The Clinical Case Manager is a member of the CFCS Congregate Care Team and provides casework and case management services to residents of CFCS’s group homes for transition age youth, 16 to 22 years, referred by the Department of Children and Families, Ascentria Care Alliance or the Department of Mental Health. The Program supports youth to:
· Develop skills and abilities needed to successfully transition to adulthood;
· Develop skills and abilities needed to live independently;
· Establish and promote lifelong connections;
· Establish and achieve educational, vocational and employment goals;
Under the supervision of the Program Director, and the clinical guidance of the Director of Clinical Services, the Clinical Case Manager (CCM), collaborates with each youth, to assess abilities, needs and challenges, identify goals, and develop an individualized treatment plan. Driven by a permanency focus, and guided by a trauma informed lens, the CCM works to strengthen the youth’s personal support network and obtain the resources and services needed for each youth to successfully achieve their goals.
DUTIES AND RESPONSIBILITIES: The Clinical Case Manager:
· Completes initial clinical and permanency assessments with each youth;
· In collaboration with each youth and the Team, identifies goals and develops treatment plans;
· Reassesses and reviews treatment plans in accordance with agency policy and procedures;
· Attends all required case review meetings mandated by the funding provider;
· Facilitates resident therapeutic groups as needed;
· Attends weekly staff meetings, supervision and monthly trainings;
· Meets individually with each youth to support successful outcomes; additional family therapy may be needed with identified life-long connections;
· Identifies resources and care needs including physical and mental health, dental care, education, employment preparation; assists youth in obtaining needed care and services and in developing skills to manage these tasks independently;
· Ensures that all staff are aware of any current concerns or plans, in accordance with agency policy and procedures;
· Establishes and maintains productive working relationships with formal and informal community resources which might assist youth;
· Maintains timely, up to date, accurate case documentation;
· Other duties as assigned by supervisor.
REQUIRED EDUCATION & EXPERIENCE:
· Master’s degree in social work or a related field;
· LCSW or equivalent professional licensure (or license eligible at hire and the ability to become licensed within 1 year of hire);
· Minimum two years’ experience providing clinical services to high-risk youth, experience in working with youth in a residential setting preferred.
· Valid driver’s license and properly insured vehicle;
· Clearance of background checks as required by state and federal law;
· Current certification in CPR and first aid.
· MAP (MA Medication Administration Program) certified or the ability to obtain it within 3 months of hire
